Congratulations! Your Application to join the Priority Wait List for an IDD Unit at Canton Oaks has been approved by the Canton Oaks HOA, pursuant to the conditions and expectations listed below.

This approval is specific to the parent or guardian that completed the Application and the Individual with IDD that will reside in the IDD Unit listed in your Application. Approval of any other individuals that may reside in the same IDD Unit will be determined at a later date.

Completing this Application Acceptance Form and paying the Program Fee are the final steps in the Application process.

By signing and submitting this form, you are acknowledging that approval of your Application includes the following conditions and expectations of you as a member of the Canton Oaks community:

• You will actively participate in the training, educational, community building, and family-readiness activities that TOC and TOCB will be offering throughout the design, development, and launch of Canton Oaks.

  
  

• You will pay the Program Fee of $795, due no later than December 15, 2025. This fee is refundable if you cancel your Application on or before February 1, 2026. Note, only one Program Fee is required per family, regardless of the number of IDD Units you are reserving or the number of IDD individuals within your family that are applying to live in an IDD Unit at Canton Oaks. If you complete the family training program and purchase or lease an IDD Unit at Canton Oaks, you will receive a $500 credit towards your HOA fees at closing.
